サービス提供範囲 (Service Scope Policy)
ReBornMe は、海外における美容医療体験を円滑にするための予約サポート・通訳・情報提供・生活支援などの非医療コーディネーションサービスを提供します。
ReBornMe は医療行為・診断行為・治療方針の決定・医師の判断への介入を一切行いません。
施術や治療の最終的な判断・説明責任は、提携医療機関および担当医師に帰属します。
ReBornMe provides non-medical coordination services, including consultation support, appointment scheduling, language assistance, and recovery support.
ReBornMe does not perform medical procedures, provide medical advice, or participate in clinical decision-making.
All medical procedures and treatment decisions are the responsibility of licensed physicians at accreditedmedical institutions.
料金・請求に関して (Payment & Billing Policy)
・ReBornMe のコーディネーション・サポート費用は、サービス開始前にご案内いたします。
・お支払いは銀行振込 / キャッシュレス決済にて対応しております。
・施術費用はクリニックへ直接お支払いください。
・看護サポート、日常生活支援、送迎などの追加サポートサービスは、
ご依頼内容に応じて別途料金が発生いたします。
・Coordination and support service fees are billed prior to service engagement.
・Payments may be made via bank transfer or cashless electronic methods.
・Medical procedure fees are paid directly to the clinic.
・Additional support services, including nursing care, daily living assistance, and transportation, would be charged separately based on the specific arrangements requested.
キャンセル・日程変更ポリシー (Cancellation & Rescheduling Policy)
サービス開始後のコーディネーション費用は返金不可となります。
クリニック側の予約変更は、各医療機関の規定に準じます。

個人情報の守秘 (Confidentiality & Privacy Policy)
ReBornMe は、お客様の個人情報・診療履歴・相談内容・連絡記録を厳重に管理します。
データは許可された担当者のみがアクセスできる状態で安全に保管され、お客様の明確な同意なく第三者へ提供されることはありません。
また、ReBornMe はフィリピンのデータプライバシー法（RA 10173）および国際的な個人情報保護基準に準拠して運用しています。
取得した個人情報は、適用される法令（RA10173 等）に加え、該当する範囲で日本の個人情報保護法（APPI）の趣旨にも適切に配慮して取り扱います。
ReBornMe protects all personal information, medical history, treatment details, and communication records with strict confidentiality.
Data is securely stored and accessible only to authorized personnel.
No personal or medical information is shared with third parties without explicit client consent.
ReBornMe complies with the Data Privacy Act of the Philippines (RA 10173) and international confidentiality standards.
Personal data is handled in accordance with RA10173 and, where applicable, with the spirit of Japan’s APPI.
医療結果に関する責任 (Risk & Responsibility Policy)
美容医療には、施術内容に応じたリスク・回復期間・個人差が伴います。
仕上がり・経過・最終結果に関する説明責任および判断は、提携クリニックおよび担当医師により直接行われます。
ReBornMe は、お客様が信頼できる医療機関と安全に繋がるためのサポートを提供しますが、医療結果について責任を負う立場にはありません。
All cosmetic medical procedures involve risks, recovery considerations, and individual variation.
Explanation of risks, expected outcomes, and final treatment decisions are made directly by the attending physician.
ReBornMe facilitates access to reputable medical providers but is not responsible for medical outcomes or results.

ReBornMe は旅行商品（航空券・宿泊・ツアー等）の販売・手配代行は行いません。
必要に応じてお客様ご自身または提携旅行事業者をご案内します。
看護師・通訳等の提供は提携医療機関または適法な事業者によって行われ、ReBornMe は人材派遣・職業紹介の当事者にはなりません。
ReBornMe does not sell or arrange travel products (air tickets, hotels, tour packages).
Clients may arrange them directly or through licensed travel providers.
Nursing and interpreting services are provided by partner medical institutions or duly licensed providers;
ReBornMe is not a labor dispatch or recruitment agency.
